Physiotherapy SOAP Notes

The physiotherapy SOAP notes, also called PT or physio Notes, is a particular format of recording patient or client information used by physiotherapists.

Most physio notes are written in the S.O.A.P note format. SOAP stands for Subjective, Objective, Assessment or Analysis and Plan. It is a record of the progress of the patient, which is included in his or her patient medical chart.

The physiotherapy soap notes are often the recording format done during client encounter after the initial evaluation or examination.
